Emergency Medical Responder Certification Package

We have streamlined our forms into distinct categories and complete packages to help you find everything you need quickly. Use the links below to download the complete package.

The packages contain the downloadable printable forms and information required for certifying. However, be sure to fill out the electronic application online HERE to save the $50 hand processing fee. You should still download the applicable package and print any supporting documentation you need. You may scan and submit the documentation online or you may mail the supporting documentation in. There is no additional fee for mailing in your supporting documentation.
